Oral Oncol. 2015；51：1041-6. （メタ）【検】CQ4-2ⅢおよびⅣA期の上咽頭癌に対して，放射線治療に化学療法を同時に併用することは推奨されるか？推　奨ⅢおよびⅣA期の上咽頭癌に対して，放射線治療に化学療法を同時に併用するよう推奨する。推奨の強さ：強く推奨する　エビデンスの確実性：A　合意率：100％解　説上咽頭癌は化学療法の感受性が比較的高く，また放射線治療（radiation therapy：RT）にシスプラチンなどの化学療法を併用することで放射線治療の効果が増強することが知られている。局所進行上咽頭癌においては, 放射線治療の前（導入化学療法, induction chemothera-py：ICT）や, 後（補助化学療法, adjuvant chemotherapy：AC）に化学療法が用いられるが, これに関してはCQ4-3に譲る。本CQでは, 放射線治療の期間において, 化学療法を同時併用すべきかどうかについて解説する。Intergroup study 0099 1）をはじめ，多くの第Ⅲ相ランダム化比較試験において局所進行上咽頭癌に対する同時併用化学放射線療法（concurrent chemoradiotherapy：CCRT）の有効性が検証された。100例以上においてRT単独と比較された主な試験を参考文献1-9）に示す。なお，これらのランダム化比較試験では検証群においてCCRTに加えてACも含まれている 1-3,5,6,8）。一部で全生存率（overall survival rate：OS）における統計学的有意差がわずかに示されていないが（Kwongら 8）：3年OS CCRT＋AC 86.5％ vs. RT 76.8％；p＝0.06），ほか172
