■参考文献1） 加藤洋一，高山忠利．【小外科・外来処置マニュアル】 今後の課題 人咬傷後にHBV，HIV，HCV抗原抗体検査をした群としていない群で比較し，曝露後感染予防で感染せずに済んだかどうかの研究は成立しないため，エビデンスレベルの高い文献が得られない。厚生労働省の肝炎総合対策ホームページには被汚染者のHBs抗原抗体の検査をし，抗原抗体とも陰性の場合，高力価HBsヒト免疫グロブリン（HBIG）1,000単位を遅くとも48時間以内の接種かつ3回のHBワクチン接種を行うことが望ましいとしている。汚染源がHBe抗体陽性の場合はHBIGのみでも感染が予防できるが，汚染源がHBe抗原陽性のときはHBIGに加えHBワクチン接種も推奨している。HCVは曝露後有効な感染予防方法はない。免疫グロブリン製剤投与は推奨されていない。曝露直後のインターフェロン投与は感染を予防できるというエビデンスがなく，針刺し事故においても副作用の問題から投与を行わない施設も多い15, 16）。厚生労働省の肝炎総合対策ホームページにも針刺し事故の感染予防に免疫グロブリン製剤は効果がなくインターフェロンは一般には行われていないとしている。治療にはインターフェロンが有効なため，感染の有無を評価するためHCV抗体検査や，HCV-RNA検査は有用である。四肢・皮膚 咬傷．臨外．59：338-9, 20042） 武田多一．診断の指針 治療の指針 虫刺され，動物咬創．綜合臨．49：949-50, 20003） Kerins M, Greene S, OʼConnor N.
