 根拠･解説 

 「鼻根部」「前頭頬骨縫合部」は堅牢な支柱部分であるため，この2カ所のプレート固

定が推奨されている

。一方，田嶋は，この2カ所のプレート固定のほか，「頬骨弓部」もプレート

固定が勧められるが，転位が軽度の場合は必要でないと述べている

。水平方向のButtressとして

の頬骨弓の重要性を強調する報告も，同部位の固定を支持している

以上のことから，Le FortⅢ型骨折では基本的に①＋②が有効であるが，頬骨弓の状態によって③

を追加することが推奨できる。

今後の課題 

 Le FortⅢ型骨折に対する骨固定法には，近年，さらなる工夫も報告されている

。

現時点では一般化された概念ではないが，今後の課題として多施設共同研究等で精度の高い検証の蓄
積が望まれる。
