##1germinoma AND classification#2germ cell tumor AND embryonal AND central nervous system#3neoplasms AND germ cell AND classification#4#3 AND central nervous system#5germ cell tumor AND tumor marker#6#5 AND central nervous system#7#6 AND diagnosis（遊離を含む）をすべて測定（トータルHCG）しているのかを意識して測定値をみる必要がある。正常な状態ではインタクトHCGとトータルHCGはほとんど同じ値であるが，腫瘍性疾患では乖離がみられる可能性がある。一般的にインタクトHCGはECLIA法を用いて測定され，単位はIU／LまたはmIU／mLである［論文においてはI（international）を省略してU／LやmU／mLとしているものもある］。一方，遊離型のβ-HCGのみを測定する場合はRIA法で測定され，単位はng／mLである。また，トータルHCGはCLEIA法で測定され，単位はIU／LまたはmIU／mLである。胚細胞腫瘍の臨床において，どの測定法が高い特異度であるかについて報告はないが，精巣の胚細胞腫瘍において，トータルHCGを測定する方が，治療効果，再発に関してより特異的であったと報告がある10）。　以上の検索式より，一次スクリーニングとして141文献を抽出し，45文献の構造化抄録を作成した。それらのエビデンス総体をもとに推奨と解説文を作成したが，その後にもいくつかの文献を加える必要があり，随時訂正し，最終的にCQ1では12文献を抽出するに至った。 ❖文　献 1） Matsutani M, Sano K, Takakura K, et al.
